CKE Team Members are welcome to sign-up for recurring or one-time contributions through payroll deduction. Complete this AUTHORIZATION FORM to initiate, change or terminate a payroll deduction, and submit the completed form by fax 615-814-8333 or email shiningstar@ckr.com.   CKE employees can also enroll by going to MyStar/Giving.
